PAYMENT & CANCELLATION POLICY
Eminence Physical Therapy is a cash-based physical therapy practice, which means that patients pay a set price up-front for physical therapy services provided. Eminence Physical Therapy does not bill insurance companies for services, however the patient may submit their own claims to their insurance company for possible reimbursement for services provided. Eminence Physical Therapy is considered an out-of-network provider for all insurance companies. Please check your insurance policy to inquire about receiving physical therapy services from an out-of-network provider to determine if you can be reimbursed for these services, although this is not mandatory.
Our cash-based model allows your therapist, Dr. Champion, to guarantee personalized one-on-one care for all services provided, which ensures the highest quality of care possible. In addition, Eminence Physical Therapy will provide you with the necessary documentation needed in order for you to file your own claims with your insurance company should you choose to seek reimbursement. Eminence Physical Therapy cannot guarantee that the insurance company will provide reimbursement for services you receive. It is the patient's choice and responsibility if choosing to submit claims.
A $50 cancellation charge applies if notiﬁcation of appointment cancellation is not received within 24 hours of scheduled appointment.
A $25 cancellation charge applies if notiﬁcation of appointment cancellation is received within 24 hours of scheduled appointment.
These fees MUST be
paid in full PRIOR to the resumption of future scheduled appointments.
